Figure 3: Post-traumatic arterio-venous fistula (AVF). Trauma six years ago followed by gradual resistant hypertension. a) Aorto-renal angiogram: arterialization of the inferior vena cava (IVC) through a wide bore AVF inside the kidney. Renal vein diameter is 5 cm. The decision was to attempt autotransplantation with consent for nephrectomy. b) Coiling was offered as an alternative. Seven electrolytically detachable long coils were critically deployed with care to stop blood flow. c) Control angiogram showed the excellent response and the preservation of the rest of the kidney. Patient has been normotensive for 18 months to date.
