I read the interesting article by Razazian et al, where the authors have compared gabapentin with levodopa-carbidopa and have found gaba- pentin to be superior to levodopa-carbidopa in patients who were on hemodialysis.[1]As rest- less leg syndrome being a common and an interesting disease entity, one is supposed to follow the new developments. Earlier one study implicated pregabalin as a better drug in patients who were not on hemodialysis.[2]Later on, there was a study that peripheral hypoxia is the main culprit and correction of the same may revert the symptoms.[3]Now, here the authors have found gabapentin as a better drug. Looking into the varied treatment responses in different studies and etiological difference in its causation such as hereditary to iron defi- ciency, uremia, peripheral neuropathy, and central dopaminergic deficiency may be the reason for this diversity. Hence, the treatment of an individual patient must be tailored according to evaluation about this diversity.
